TooleyCDG’s Homelessness Assistance programs work with individuals and families experiencing literal homelessness as defined by the U.S. Department of Housing Urban Development (HUD) in CFR 24 578.3.

Here are three ways TooleyCDG can help:
Case Management

Our Case Managers work diligently with clients toward self-identified housing goals. The process of gathering documents and attaining income to support permanent housing can take up to six months. Steps may include:

  • Assistance with SSI/SSDI applications
  • Help to acquire documentation needed for stable housing
  • Developing an individualized step-by-step case plan.
  • Referrals to healthcare and recovery counseling

Permanent Housing

For eligible clients, meeting the HUD definition of homelessness (categories 1 & 4) TooleyCDG can provide the financial assistance necessary to attain permanent housing. Financial assistance can include:

  • First Month’s Rent
  • Security Deposits
  • Utilities Deposits
  • Cost of replacing necessary documentation
  • Moving Expenses

Essential Supplies

We understand that the process of attaining permanent housing may be a long road. While clients work with their Case Managers on their housing plan, we can help make the wait easier by providing essential supplies such as:

  • Food & Water
  • Blankets & Sleeping Bags
  • Air Mattresses
  • Tents
  • Bus Passes
  • Outpatient healthcare services
  • Prescription Medications
Understanding the Process

01

Intake Appointment

~ Two Hours

You will be scheduled to go through an in-depth intake with our specialist who will ask a lot of (sometimes very personal) questions to assess your eligibility for the program. You can come to us or we can come to you or meet you at a safe, convenient place. The intake appointment can take up to two hours.

HMIS Enrollment

(Done During Intake)

If you are determined eligible, with your permission, you will be entered into the Homeless Management Information System (HMIS) as a client. Brevard Homeless Coalition will match you with a service provider. That may be TooleyCDG or another partner agency.

Case Management

~3-6 mos.

If you are referred to us by the Brevard Homeless Coalition, we will call you for an appointment with one of our Case Managers. They will finish up your file by asking for documentation – it’s ok if you don’t have it our staff will help you get replacements.

Housing Identification

~2 – 3 mos.

Once your file is complete, and your Case Manager determines you are ready, you will speak with our Housing Manager to identify housing options in your price range. (Having income creates more options and a shorter process.)

Negotiation & Inspection

~2-3 weeks

Once housing is identified the Housing Manager will negotiate with the landlord, conduct an inspection, and determine how much financial support you may need – we are permitted in some circumstances to pay up to three months’ rent.

On-going Case Management

1 Year

After getting you stably housed, your case will remain open for a year. We will check in with you monthly to make sure everything is ok. If more support is needed, we’ll work with you. If not, after a year, as long as you are stable, we’ll agree together to close your case.

Successful Client/Case Manager Relationship

Clients Responsibilities

  • Participate in the development and execution of your case plan
  • Be consistent working your plan and communicating with your case manager
  • Understand the steps of the process and how your participation is required for success

TooleyCDG Responsibilities

  • Provide clear, consistent communication
  • Work with you to develop an individualized housing plan
  • Be an advocate for you with partner agencies
  • Treat every client with kindness, respect, and dignity
